Most supplements marketed as adaptogens are backed by animal studies, theoretical mechanisms, and aggressive marketing. Ashwagandha is different — and if you have never looked at the controlled trial data, you may be underestimating it.

The research on KSM-66, a standardised root extract, covers three distinct outcomes in athletic populations: cortisol reduction, muscle strength gains, and aerobic capacity improvement. All three have randomised controlled trials. Not all three effects are equally large, but all three are real.

The foundational cortisol study came from Chandrasekhar and colleagues (2012, Indian Journal of Psychological Medicine). In a double-blind RCT, 64 adults received 300mg KSM-66 twice daily or placebo for 60 days. Serum cortisol dropped by 27.9% in the treatment group versus 7.9% in placebo — a statistically significant and clinically meaningful difference. The Perceived Stress Scale score also fell by 44%, suggesting the cortisol reduction translated to subjective experience.

For athletes in heavy training blocks, this matters. Cortisol chronically elevated above the physiological baseline suppresses testosterone synthesis via HPA axis negative feedback, increases muscle protein catabolism through glucocorticoid receptor activation, and blunts the IGF-1 signalling downstream of GH pulses. Reducing chronic training-related cortisol elevation is not a trivial outcome.

The most cited strength RCT is Wankhede and colleagues (2015, Journal of the International Society of Sports Nutrition). Fifty-seven young men performing resistance training received 300mg KSM-66 twice daily or placebo for 8 weeks. The ashwagandha group gained significantly more muscle mass across chest, arms and legs, increased bench press 1RM by 46kg vs 26.4kg in placebo, and recorded higher serum testosterone (+96 vs +18 ng/dL) alongside lower creatine kinase post-exercise — indicating reduced muscle damage. Testosterone elevation was modest but directionally consistent across multiple subsequent trials.

Importantly, Wankhede controlled for training volume and protein intake — the groups did not differ on either. The effect appears to be mediated via withaferin A and withanolides (the bioactive steroidal lactones in the root), which may upregulate LH secretion and reduce SHBG — effectively raising free testosterone fraction.

The aerobic capacity finding is perhaps the most surprising. Choudhary and colleagues (2017, AYU Journal) randomised 50 healthy adults to 300mg KSM-66 twice daily or placebo for 12 weeks. VO2max improved by 4.91 mL/kg/min in the ashwagandha group versus 1.42 mL/kg/min in placebo — a significant difference. The mechanism is likely indirect: reduced physiological stress load and improved sleep quality (ashwagandha shows consistent effects on sleep onset and slow-wave sleep proportion) allow greater training adaptation rather than directly stimulating mitochondrial biogenesis.

Protocol note: The vast majority of positive RCT evidence uses 300mg standardised root extract (5% withanolide content) taken twice daily. Lower doses and leaf extracts have substantially weaker evidence.

Side effects are mild and rare in the published literature — primarily GI discomfort at high doses. Ashwagandha is contraindicated in pregnancy (uterotonic effects) and should be used cautiously with thyroid medication, as it modestly upregulates T4 and T3 in hypothyroid populations (Sharma 2018).
